Annex Request Form

The Annex is a remote storage facility for library materials. Use this form to request that materials be delivered from The Annex to an on-campus library. Requests are delivered to on-campus libraries twice daily, Monday - Friday. For the morning delivery, please submit a request by 9 a.m. For the afternoon delivery, please submit a request by 1 p.m. Library patrons will be notified by e-mail when the items are scheduled for delivery. Materials will be held at the on-campus library for seven days. The Annex houses fragile or unique items, and special handling may be required for some materials.
