Annex Request Form

Use this form to request books that are housed at the Annex.

Enter your requests for library materials housed at the Annex in the areas provided to the right. The library messenger retrieves Annex materials twice each day (Monday through Friday) and delivers requested items to the owning library. For example, items with Acorn locations saying CENTRAL ANNEX will be delivered to the Central Library circulation desk; items with DIVINITY ANNEX locations will be delivered to the Divinity Library circulation desk. Materials will be held for 7 days from the date requested.
Patrons requesting material be sent to the Central and Music Libraries will receive email notifications from Annex staff indicating shipment times. For all other libraries, patrons will Only be notified by Annex staff if there is a problem in retrieving their request.
